Each patient is supported by a dedicated care team – including a primary care provider, nurse, clinical pharmacist, and specialists – working together in an integrated model. Senior Care primary care services are designed to be a value-based model and work best when paired with Medicare Advantage plans.

Our Senior Care program uses an integrated approach, so we coordinate all aspects of your health, including your medications.
