At Brevard Health Alliance, Advanced Care Plus (AC+) provides personalized, coordinated primary care designed for adults with Medicare. Our dedicated team—including primary care providers, nurses, clinical pharmacists, care coordinators, and other healthcare professionals—works together to help you manage your health, navigate changing healthcare needs, and get the most from your care.
From managing chronic conditions and medications to coordinating specialist care and connecting you with additional services and resources, Advanced Care Plus is focused on helping you stay healthy, active, and independent.

Advanced Care Plus (AC+) is a comprehensive primary care program designed to give adults with Medicare more personalized, coordinated healthcare. Your dedicated care team works together to manage your health, coordinate care with specialists, support you between visits, and help you navigate changing health needs as you age.
Advanced Care Plus is designed for adults with Medicare who would benefit from a more coordinated approach to primary care. If you’re interested in the program, our team can review your healthcare needs and insurance coverage and help determine if AC+ is right for you.
Uninsured patients over 65 may qualify for the AC+ program and reduced fees based on household size and income through our Sliding Fee Discount Program.
There is no additional fee to enroll in Advanced Care Plus. Services are billed to your insurance based on your individual coverage and benefits.
Advanced Care Plus provides more than routine office visits.
Your care team includes a provider, dedicated nurse, clinical pharmacist, and care coordinator who work together to:
- Coordinate specialty care
- Manage medications
- Help prevent hospitalizations
- Connect you with community resources
- Support your long-term health goals
Advanced Care Plus has a dedicated team of providers who specialize in caring for older adults. If your current primary care provider is not part of the AC+ team, we can help you transition to an Advanced Care Plus provider and make the process as seamless as possible.
Yes. Advanced Care Plus is designed to help coordinate your care, not replace the specialists you already see. Your AC+ team can communicate with your specialists, help coordinate appointments and follow-up care, and make sure your healthcare team is working together toward your health goals.
Managing medications can become more complicated as your health needs change. Your Advanced Care Plus clinical pharmacist reviews your medications to make sure they are working safely together, identify medications you may no longer need, explore more affordable alternatives when available, and answer questions about your prescriptions.
Your pharmacist works directly with the rest of your care team, so medication management is part of your overall care plan.
We don’t want transportation or mobility challenges to prevent you from getting the care you need. When clinically appropriate, Advanced Care Plus can provide care in your home and help coordinate other resources to support your healthcare needs.
Advanced Care Plus brings many of the services older adults may need together through one coordinated care team. Depending on your individual healthcare needs, services may include primary and preventive care, chronic disease management, clinical pharmacy, nutrition counseling, behavioral health, podiatry, dental care, home care, laboratory testing and bloodwork, EKGs, pulmonary function testing, trigger point and joint injections, dermatologic procedures, and care coordination with specialists.
