What Is Advanced Care Plus?
Advanced Care Plus is a comprehensive primary care program designed specifically for adults with Medicare. Rather than treating each appointment as a separate visit, our team works together to coordinate every aspect of your healthcare.
Your care team gets to know you, communicates with your specialists, helps manage your medications, and creates a personalized care plan focused on keeping you healthy—not just treating illness when it happens.

Sean Claeys, DO
INTERNAL MEDICINE PHYSICIAN
“I enjoy helping patients stay active and independent by building long-term relationships and creating care plans that fit their lives, not just their diagnoses.”
Dr. Sean Claeys specializes in helping older adults manage complex chronic conditions, including heart disease, diabetes, hypertension, and chronic kidney disease. He is passionate about preventive care and partnering with patients to help them stay healthy, active, and independent. Outside the clinic, he serves as an adjunct professor at the Burrell College of Osteopathic Medicine at Florida Tech, mentoring the next generation of physicians.

Hui-Ping Chang, ARNP

“I enjoy caring for older adults because every patient brings a lifetime of experiences that shape what matters most to them. I value building relationships and working together to help each person stay healthy, active, and independent.”
Hui-Ping Chang is passionate about helping patients achieve their best health through compassionate, evidence-based care. She has a special interest in family medicine, women’s health, and caring for older adults, partnering with patients to support healthy aging and long-term wellness. Outside of work, she enjoys cooking, traveling, and hiking, and values learning from new cultures and experiences.

Thomas Milbourne brings experience in primary care, internal medicine, geriatric care, gastroenterology, and urgent care to his work with older adults. He is passionate about preventive care and chronic disease management, partnering with patients to navigate complex health needs and help them maintain their health, independence, and quality of life as they age.

Beth Morlang, RD, CDCES, LDN

“As we age, our nutritional needs can change, especially when navigating a new diagnosis or health concern. I help make good nutrition approachable while still helping people enjoy the foods they love.”
Beth Morlang believes good nutrition should be practical, approachable, and fit into real life. With clinical interests in chronic kidney disease, diabetes management, and patient education, she helps patients understand how food choices can support their health while navigating complex healthcare needs. Outside of work, Beth enjoys discovering great food, exploring local destinations, listening to everything from Mozart to Metallica, and laughing with friends.
